I paid $200, although their website is now quoting the price of an assessment at $400. So either I got a deal (I think they do run periodic "specials"), or they've jacked up the price since I registered in August. (I developed trochanteric bursitis a week before my originally scheduled appointment, and ended up having to postpone it for quite a while.)
